Detailed Care Assessment

Once you have met with a team member and the assessment has been completed, and the costs and level of care have been discussed and agreed, we would then send you a profile of our recommended carer. Suppose you are happy with the carer profile, in that case, we will arrange for a telephone call or a video call via Zoom or Skype so that you can ask questions and better get to know our proposed carer.

Meet Your Carer

Once you have met with a team member and the assessment has been completed, and the costs and level of care have been discussed and agreed, we would then send you a profile of our recommended carer. Suppose you are happy with the carer profile, in that case, we will arrange for a telephone call or a video call via Zoom or Skype so that you can ask questions and better get to know our proposed carer.

All of our carers work directly for us, are trained and managed by us, so you have peace of mind. You don’t need to worry about engaging the carer directly, taking care of insurance and taxes, etc., as you would if you obtain live-in care through an Introduction Agency.

Your Live-In Care Begins

The last step will be completing all the final details, including the start date. Once care starts, access to our OneTouch system will be given to you, which allows real-time access to your care records, which helps your family or loved ones keep up to date with your care if you want to grant them access.

Anyone involved in your care (either yourself, a family member, or a member of your MDT such as your case manager, discharge team, social worker, etc.) can contact our professional team to discuss your requirements and any questions you have regarding the care service we offer.
If you want to proceed further, we will schedule a no-obligation visit in a convenient place for you with a member of our Complex Care team. This might be at home, or in an alternative setting such as hospital or residential setting.
This will be an opportunity for us to complete a detailed care assessment and evaluate your needs. Together with relevant health and social care professionals we will build a bespoke care plan specific to your individual needs and requirements.
Once your assessment has been completed and level of care has been agreed, we then send you profiles of recommended carers. You will have the opportunity to meet with your proposed carer to ask questions and get to know them a little better. When you are happy to proceed, we complete the final details and arrange a start date.
